Bantuan Sejahtera Pesakit Terlantar (BSPT) merupakan satu inisiatif yang disediakan oleh Kerajaan Negeri Kedah bagi membantu pesakit kronik yang terlantar. PENGENALAN BANTUAN TUNAI RM500 UNTUK PESAKIT TERLANTAR BSPT
Bantuan Sejahtera Pesakit Terlantar (BSPT) merupakan satu inisiatif yang disediakan oleh Kerajaan Negeri Kedah bagi membantu pesakit kronik yang terlantar. Tujuan BSPT ini ditubuhkan adalah untuk meringankan beban keluarga dan penjaga. Bantuan ini juga dapat menggalakkan keluarga atau penjaga untuk terus menjaga ahli keluarga mereka yang sakit atau terlantar dan menggalakkan oku atau pesakit kronik untuk terus tinggal bersama keluarga iaitu penjaga. Bantuan ini juga sedikit sebanyak dapat membantu memelihara dan memperkukuhkan institusi keluarga.
KADAR BANTUAN BSPT
Kerajaan Negeri Kedah menyediakan bantuan kewangan berjumlah RM500.00 secara sekaligus kepada:
- Setiap Pesakit Kronik;
- Penjaga kepada Pesakit Kronik;
- Pesakit Kronik Terlantar; atau
- Pesakit Kronik OKU Terlantar.
KRITERIA KELAYAKAN BSPT
Bagi mendapatkan bantuan dan memastikan mereka yang betul-betul LAYAK sahaja untuk menerima bantuan ini, beberapa kriteria kelayakan perlu dipatuhi seperti:
- Merupakan warganegara Malaysia dan haruslah bermastautin di negeri Kedah;
- Merupakan pesakit kronik atau pesakit yang terlantar oleh kerana sakit yang berkekalan, tidak dapat diubati atau dipulihkan sehingga hilang mata pencarian; atau
- Pesakit terlantar yang boleh diubati atau dipulihkan yang memerlukan rawatan susulan namun menjejaskan mata pencarian; atau
- Menghidap penyakit kronik yang memerlukan penjagaan atau pengawasan yang berterusan.
CARA UNTUK MEMOHON
Pemohon yang ingin mendapatkan bantuan tunai RM500 ini, boleh dapatkan borang permohonan melalui pihak yang berikut:
- Semua Pejabat Kebajikan Masyarakat Daerah – Klik sini untuk direktori
- Semua Pejabat Penghulu Mukim – Klik sini untuk lihat Senarai Penghulu Mukim
- Semua Pusat Khidmat Wakil Rakyat / Penyelaras DUN – Klik sini untuk senarai penuh Pusat Khidmat Wakil Rakyat / Penyelaras DUN
Pastikan pemohon bawa bersama dokumen sokongan yang dinyatakan di bawah ini semasa mendapatkan borang permohonan, lengkapkan borang tersebut dan sertakan bersama dokumen sokongan kepada pihak Pejabat Kebajikan Masyarakat Daerah/Pejabat Penghulu Mukim/Pusat Khidmat Wakil Rakyat/Penyelaras DUN.
DOKUMEN YANG DIPERLUKAN
Berikut adalah dokumen penting atau sokongan yang diperlukan untuk mereka semak kelayakan pemohon, antaranya:
- Salinan Kad Pengenalan Pemohon;
- Salinan Kad Pengenalan Pesakit (sekiranya pemohon adalah penjaga);
- Salinan Kad OKU / Slip Pendaftaran Sementara OKU (jika perlu);
- Laporan Perubatan / Memo Pegawai Perubatan daripada Hospital / Klinik Kesihatan Kerajaan;
- Salinan Bil Utiliti seperti bil air/elektrik;
- Penyata Akaun Bank terkini yang mash aktif; DAN
- Gambar berwarna pemohon/pesakit (sekiranya pemohon adalah penjaga).
Setelah mengisi borang, pemohon perlu menyerahkannya bersama dokumen yang diperlukan.
